Be it Janeway accidentally ending up with the cast on her ship because of Time Sewer malfunctions, or Charles trying to get Tuvok to laugh. Maybe the historical paramedics confuse the Doctor with their addiction to honey?

I'm so sorry for this XD (But you have to wonder what Charles WOULD do with a holodeck...)
